Application Process
To be considered for future opportunities with GILK, please follow these steps:
1. Complete the Trade Contractor Pre-qualification Statement:

Email the completed Pre-qualification Statement to our accounting department at accounting@gilk.com.
Insurance Requirements
All trade contractors must maintain the following insurance coverage:
1. Liability Insurance
$1,000,000 per occurrence
$300,000 for Damage to Rented Premises
$10,000 Medical Expense per person
$1,000,000 Personal and Advertising Injury
$3,000,000 General Aggregate
$3,000,000 Products Completed Aggregate
Additional Insured: GILK must be named as an additional insured for operations and completed operations. This can be on a blanket basis or through a specific endorsement.
Primary & Non-Contributory: The insurance certificate must state that the coverage is primary and non-contributory.
Waiver of Subrogation: The General Liability policy must include a waiver of subrogation in favor of GILK.
Insurance Company Rating: Each policy must be issued by an insurance company with a minimum rating of "A-VII" or better.
2. Workers’ Compensation Insurance
Required if You Have Employees:
Employers Liability Limits:
$500,000 disease per employee
$500,000 disease policy limit
Waiver of Subrogation: Must include a waiver of subrogation in favor of GILK.
Certification Requirement: A valid certificate of workers' compensation insurance is mandatory. Contractors without this certificate will not be allowed on a GILK job site.
Additional documentation
Safety and Health Program:
A written Safety and Health Program must be on file with GILK.
A sample program is available on OSHA’s website.
A copy of the completed W-9 must be on file.
